After thirty years of solo Plastic Surgery practice, Kevin B. Mayfield, M.D., F.A.C.S. has permanently closed his office at 8230 Beckett Park Drive in West Chester, Ohio.


Written requests for copies of medical records should be sent via email to drkevinmayfield@icloud.com . Please include your contact information in the correspondence and you will be contacted regarding this request. Allow 6-8 weeks per request after a release of medical information form has been signed and returned. All appropriate fees set by the State of Ohio will apply and must be prepaid.
